Brief, concrete description of his feat of arms or accomplishments:
Seaman S.I. Zamotkin served in the Patriotic War from August 25, 1944 onward. During his service in the Northern Fleet comrade Zamotkin directly participated in his ship’s combat missions. He was involved in the ship’s transfer from the USA to the USSR. During this journey the ship was more than once caught in extremely severe force-nine ocean gales. During one of these gales the auxiliary engine’s electric parts were put out of action. Under arduous conditions and within a short time comrade Zamotkin managed to repair the damage, thus bringing the ship back to a state of combat readiness. In the rough waters of the Barents Sea comrade Zamotkin showed himself a bold, brave, and steadfast sailor. He was directly involved in 23 combat missions during which the ship escorted Soviet and allied convoys and 18 combat missions during which the ship hunted for enemy submarines. Comrade Zamotkin’s equipment was always in excellent condition.
Comrade Zamotkin repeatedly took part in landing operations, during which he showed examples of bravery and heroism. When a number of scouts were put ashore in the Bolshaya Volakovaya Bay, he personally helped them disembark. He carried weapons and ammunition on his shoulders.
When the ship was transferred from the Northern Fleet to the Red Banner Baltic Fleet comrade Zamotkin performed a number of important Government-ordered tasks, toiling heroically to construct a floodgate in the Sviri-3, for which he received a number of commendations from his commanding officer.
For his heroism, courage, and bravery in the struggle with the German invaders, I hereby nominate comrade Sergei Ilyich Zamotkin, pursuant to the Statute on Orders and Medals, for the Medal of Ushakov.
Commanding officer of BO-220, 7th Large Submarine Hunter Squadron, Defensive Sea Area Command, Southwestern Naval Defense Area, Red Banner Baltic Fleet
Lieutenant [signed] /Bashev/
