Short translation for the citation for the Order of Glory 3rd class:

In the battles for the Socialist Motherland on November 21, 1943, in the area of the village of Lenino near the town of Radomyshl, anti-tank rifleman Pyatak N.V. showed an example of courage and steadfastness on the battlefield.

While repelling 4 enemy counterattacks, Comrade Pyatak worked simultaneously with both an anti-tank rifle and a standard rifle. He destroyed 16 Hitlerites (enemy soldiers) and suppressed the fire of 2 enemy machine guns.

For exemplary performance of command assignments on the front of the struggle against the German invaders and the valor and courage displayed therein, I recommend Comrade Pyatak for a government award.

Short translation for the citation for the Medal for Bravery #931671:

For the fact that during the repulsion of an enemy attack at the edge of the forest in the area of the village of Dobrye Lozy, he used his anti-tank rifle to destroy an enemy machine-gun nest.

Short translation for the citation for the Medal for Bravery #2009945:

In the battles for the Socialist Motherland near Chepelyuvka in the area of Guta-Dobryanskaya, Comrade Pyatak demonstrated an example of steadfastness, bravery, and mastery of his craft.

Comrade Pyatak, together with his loader Shlykov A.K., used their anti-tank rifle to knock out 2 German medium tanks.

For the valor and courage displayed, I recommend Comrade Pyatak for a government award.
